Geriatric Case Manager

The NKADD, is seeking a qualified individual for a full-time Geriatric Case Manager. This position will work with geriatric populations. Responsibilities include assessing eligibility and providing ongoing case management for state and federal services. Services can include Home Delivered Meals, Personal Care, and Homemaking. The position functions also include the use of a database, offsite office work, and the organization and oversight of some events. Candidate must have exceptional communication and writing skills and knowledge of community resources. Excellent benefits and generous retirement plan. Position open until filled. Starting salary range from $35,000 to $41,000 DOQ. EOE. TDD 711.
Requirements:
- Bachelor’s degree in Social Work or related Human Service Field or RN is required.
- One year experience in health or human services; or the educational or experiential equivalent in the field of aging or physical disabilities.
- A valid driver’s license is necessary to be considered.
- Reliable transportation and the ability to complete in-home visits throughout an eight-county region are also required.
- Must possess excellent analytical, problem-solving, and decision-making skills as well as sound judgment.
- Must have the ability to understand and use job-specific software as well as a level of comfort with the use of laptops and computers.
